Privacy Policy (politica) discloses Health Connection’s privacy practices and applies to our customers’ personal data, received through our reception channels described below.If you do not agree with the content of this policy, please do not provide us with your personal data.By accepting this Privacy Policy (politica), you fully agree with its terms and consent, freely and expressly, for Health Connection to process your information, following strict international security and protection standards, such as the ISO27001 Security Management Standard of Information and requirements of the General Data Protection Regulation (GDPR) of the European Community.
1. DEFINITIONS:
- “Personal Data” is information that can be used to identify you, directly or indirectly, alone or in conjunction with other information. This includes information such as your full name, email address and telephone number, among others.
- “Sensitive Personal Data” is data that reveals racial or ethnic origin, political opinions, religious or philosophical beliefs, trade union membership, genetic data, biometric data for the purpose of unambiguous personal identification, data relating to health status or sexual life and/or or sexual orientation of a person.
2. RECEIPT OF DATA:
Health Connection uses the following channels to receive and create data from Customers:
a) Questionnaire delivered for completion by Customers.
b) Website www.saudecoluna.com, in the options Book an Appointment/First Appointment and Learn More.
c) Direct mailing to geral@saudecoluna.com.
d) Social networks Facebook and YouTube.
e) Internal systems for recording and monitoring treatment.
The following Personal Data may be received and created:
a) Full name.
b) Birth date.
c) TIN.
d) Complete address.
e) Marital status.
f) Number of children.
g) Profession.
h) Company.
i) Telephone work.
j) Home phone.
k) Cell phone.
l) Email.
m) Photos.
The following Sensitive Personal Health Data may be received and created:
a) Query reason.
b) Expected goals.
c) Areas, types, and characteristics of pain.
d) Habits and Behaviors.
e) Medical diagnoses.
f) Record of treatment evolution.
Health Connection processes children, always with the consent of the parents, and, in this way, treats the Personal Data and Sensitive Personal Data of children with the same protection and security as adult data.
3 USES OF DATA AND CONDUCT RESPONSIBILITIES:
- Health Connection processes Personal Data and Sensitive Personal Data (collects, creates, stores and consults), with the purpose of diagnosing spinal problems and identifying the best way to correct them, allowing our professionals to monitor the evolution of their patients. And manage to give an effective sequence in their chiropractic treatment, so that they become healthier and with a better quality of life.
- Health Connection also uses Personal Data to contact its customers, to book/confirm an appointment, schedule lectures, send an email for exams and other information regarding treatments and clinic events, congratulations, or information about other acts.
- No data will be shared with third parties. Exceptions are in cases where the information is required by law or court order or where there is explicit consent from the Customer holder of the data.
- All Health Connection employees who have access to Customer data are required to maintain their confidentiality.
- All employees are disciplinary responsible for the violation or illegal transmission of data received. This responsibility will be investigated through a disciplinary procedure that may culminate in sanctions provided for in the Labor Code.
4. DATA PRIVACY AND SECURITY:
- Protecting your privacy is very important to us and is part of our policy (politica). Health Connection values the protection of the information entrusted to us. We are fully aware that this data belongs to our customers, and we will use them specifically in accordance with what has been consented.
- We implement appropriate technical and organizational safeguards to protect against unauthorized or unlawful processing of Personal Data and Sensitive Personal Data and against accidental or unlawful destruction, loss, alteration, unauthorized disclosure, or improper access.
- Only employees and doctors who need the information to carry out the diagnosis and treatment process have access to Personal Data and Sensitive Personal Data, which are stored and managed in secure technological environments.
- We inform you, however, that we cannot fully eliminate the security risks associated with the processing of Personal Data and Sensitive Personal Data. In the unlikely event of a failure in our security, the competent authorities will be informed within 72 hours and, if your data is involved in the failure, you will also be informed (politica).
- By accepting this document, the Customer acknowledges and agrees that Health Connection will disclose its data, when required by public authorities, to satisfy any applicable legislation, regulation, judicial process, or governmental request.
5. CLIENT RIGHTS:
Customers may, at any time:
- Withdraw consent to the processing of informed and created data.
- Request information from stored data.
- Request the change or correction of stored data.
- Request the permanent deletion of all your Personal Data and Sensitive Personal Data.
- Requests must be made by email (dpo@saudecoluna.com),telephone (+351 214 101 429) or sending a written request to the address Av. José Gomes Ferreira, Ed. Atlas I, Miraflores, Portugal. Our opening hours are from Monday to Thursday, from 8 am to 5 pm.
- Requests will be answered within a maximum period of 30 (thirty) days, always after proof of the applicant’s identity.
- Due to future return cases, Health Connection chooses to keep the data stored for an indefinite period, aiming at access to the history and the possibility of effective continuity of treatment in cases of return. The customer may opt, at any time, for the definitive exclusion, according to the process defined in item 5 of this policy.
6. PRIVACY (politica) POLICY UPDATE:
- Health Connection may update this Privacy Policy from time to time. When this happens, the issue date and version of the policy will change accordingly, and we will only notify you of significant updates.
- The continuity of access and use of our services after the updates, characterizes the agreement and commitment with the new policy.
